Information on the Processing of Personal Data

I hereby acknowledge that the company:

(hereinafter referred to as the "Controller")

processes my personal data to the extent of: first name, last name, telephone number, and e-mail address, for the purpose of contacting me and providing the Controller's related services, in accordance with Article 6(1)(b) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation, hereinafter referred to as the "GDPR").

I also acknowledge that, until the GDPR became effective (i.e. until 25 May 2018), personal data were processed in accordance with Act No. 101/2000 Coll., on the Protection of Personal Data and on Amendments to Certain Acts, as amended.

I understand and consent to the Controller providing the personal data supplied by me (to the extent specified above) to other data processors where necessary for further processing. Such processors process personal data on behalf of the Controller and provide adequate guarantees for appropriate and lawful processing of personal data, including protection through suitable technical and organizational measures against unauthorized or unlawful processing and against accidental loss, destruction, or damage.

The personal data provided to the Controller will be processed for the duration of the contractual relationship with the Controller and subsequently for the period required for record retention in accordance with the applicable legal regulations of the Czech Republic.

I further declare that I am aware of my right to receive information regarding the processing of my personal data (in particular information about the categories of personal data processed, the purpose, method, duration of processing, and any recipients of the personal data), my right to request access to my personal data from the Controller, my right to request the rectification or erasure of my personal data (especially inaccurate, incomplete, or outdated personal data), and my right to lodge a complaint with the supervisory authority. I also understand that, as of 25 May 2018, I have the right to data portability, the right to have my personal data transferred to another controller, and the right to request restriction of processing.

I also declare that the personal data I have provided are true and accurate, that I have provided them freely, knowingly, and voluntarily, and that I understand that providing such personal data is necessary for the provision of the above-mentioned service.
